I hereby authorize Loud Production Group to thoroughly investigate any and/or all of my references, work record, education and other included in this job application to my suitability for employment and, further, authorize any prior or current employers and references I have listed to disclose to Loud Production Group any and all letters, reports and other information regarding to my work records, without giving me prior or advance notice of such disclosure. I also hereby release Loud Production Group, my former employers, and all other persons, corporations, partnerships and associations from any and all claims, demands or liabilities arising out of or in any way related to such investigation or disclosure.

If hired, I understand and agree that my employment with Loud Production Group is at-will, and that neither I, nor Loud Production Group shall be required to continue an employment relationship within any direct or specific terms. I also understand that Loud Production Group or myself may terminate the employment relationship at any time, with or without reasoning, and with or without advance notice. I understand that the at-will status of my employment cannot be amended, modified, or altered in any way by any oral modifications and must be in writing by any necessary parties.

I understand that safety of employees is of the utmost important to Loud Production Group and that the company is committed to ensuring a safe working environment. I also understand that I, and every employee, have a responsibility to prevent accidents and injuries by observing all safety practices and protocols, and follow any directions of my site supervisor(s). I understand and agree to comply with federal, state, and local regulations related to on-the-job safety and health.

I understand that should I be hired by Loud Production Group, it will be necessary for me to provide sufficient evidence of my personal identity and legal authority to work within the United States, and that federal immigration laws require me to complete an I-9 Form as evidence for lawful employment.

If hired, I understand and agree that the first ninety (90) days of employment would be on a trial basis and would be considered a continuation of the employment selection process. This ninety (90) day introductory period grants Loud Production Group an opportunity to observe and determine the capacity of myself as an employee, which includes the employee’s ability to satisfactorily perform any and/or all necessary functions of their job; and to observe and evaluate the employee’s work habits and conduct, including attendance, production output, and the employee’s relationship with coworkers, superiors, and any vendors and clients at any job site. During this introductory period, Loud Production Group may terminate employment immediately, with or without cause and with or without advance notice. Additionally, the employee may also terminate his or her employment with Loud Production Group at any time, with or without advance notice and with or without cause. This 90-day introductory period would not be a term of employment and would not be intended to, nor does it, impact the at-will nature of the employment relationship between Loud Production Group and the employee.

I understand that should any provision, portion, or term within this Agreement is declared void or unenforceable, it shall be removed while the remainder of the Agreement shall be enforceable.

I agree that all answers presented by me are factual and correct to the best of my knowledge. I further certify that I, the undersigned applicant, have personally completed this application for employment. I agree and understand that any omission or misstatement of any and all material facts on this job application or on any document used to secure employment with Loud Production Group shall be grounds for rejection of this application or for immediate job termination should I be employed, regardless of any time elapsed between application submission and discovery.
